Transition Planning

The years around your child’s transition out of high school bring some of the biggest shifts in the whole plan — school-based services end, adult day and vocational programs may have their own waitlists, benefits often need to be re-evaluated, and guardianship decisions typically need to be in place before your child turns 18.
